\n Salary: $105,000 - $110,000, plus a $15,000 sign-on bonus.
\n Location: Manhattan, NY 
\nWe also have roles open Queens and the Bronx 
\nFriday, Saturday and Sunday - 13hr shifts

\n Weekend Field RN Role Overview
\nThe Field RN/Coordinator of Care is a vital member of the home care team, managing all aspects of patient care. This full-time, permanent position follows a Monday through Friday schedule, offering stability and flexibility. Responsibilities include conducting home visits, developing and implementing care plans, coordinating with interdisciplinary teams, and ensuring accurate and timely documentation.

\n Weekend Field RN Key Responsibilities \n

\n
• \nPerform home care assessments to determine eligibility and develop care plans.\n
\n
• \nProvide skilled nursing care, including wound care, medication management, and disease education.\n
\n
• \nCoordinate and communicate with patients, families, physicians, and interdisciplinary teams.\n
\n
• \nComplete documentation in HCHB clinical software within required timeframes.\n
\n
• \nSupervise and orient home health aide personnel as needed.\n
\n
• \nIdentify needs for additional services (e.g., PT, OT, MSW, speech therapy) and coordinate accordingly.\n
\n
• \nMonitor patient progress, adjust care plans as needed, and participate in case conferencing.\n
\n
• \nEnsure compliance with infection control standards and agency protocols.\n
\n

\n Weekend Field RN Requirements \n

\n
• \nRegistered Nurse (RN) licensed in New York State.\n
\n
• \nMinimum of 6 months clinical, hospital experience.\n
\n
• \nVehicle required for most territories.\n
\n
• \nFamiliarity with EMR systems; experience with HCHB is a plus.\n
\n
• \nCurrent physical examination.\n
\n

\n Weekend Field RN Benefits: \n
