Job Title: RN - Wound Care Nurse (Home Care)

Location: Manhatten

Pay : $75.00 per hour

Job Type: Full-Time

Overview:

We are looking for a compassionate Wound Care Nurse to join our Home Care team! This is a fantastic opportunity for a certified and skilled Registered Nurse (RN) with a passion for wound care. While 70% of the role will be in the office , the nurse may also be required to travel to patients’ homes in Manhattan (MA), Brooklyn (BKLYN), Bronx (BX), and Queens (QU) to complete wound care consults.

Key Responsibilities:

• In-Office & Home Visits: 70% of the role will be based in the office, with occasional travel to patients' homes in Manhattan, Brooklyn, Bronx, and Queens to conduct wound care consults.
• Conduct health assessments with a focus on wound, ostomy, and continence care, creating individualized care plans based on patient needs.
• Recommend appropriate treatments, medications, and supplies, ensuring the best possible care for patients.
• Document patient responses to wound care, ostomy, continence, and complex wound treatments.
• Collaborate with the healthcare team, making referrals as needed for additional care.
• Educate patients and families on how to prevent complications and care for wounds and ostomies.
• Lead staff education programs, including orientation, in-service training, and continuing education.
• Assess patients at risk for pressure injuries and initiate preventive measures.
• Work closely with the Assistant Director of Nursing for Wound Care to initiate best practices and quality improvement efforts.

Qualifications:

• Education:
• Active New York State RN License
• AHA BLS Certification
• Minimum 2 years of Med/Surg experience
• Wound and Colostomy Care experience

Certifications (One of the following required):

• Certified Wound Ostomy Continence Nurse (CWOCN)
• Certified Wound Ostomy Nurse (CWON)
• Certified Wound Care Nurse (CWCN)
• Certified Wound Specialist (CWS)
• Wound Care Certified (WCC)

Additional Skills & Qualities:

• Strong assessment skills with the ability to create and implement individualized care plans.
• Must be Wound Care Certified (CWOCN, CWON, CWCN, or similar certification).
• Ability to work independently, while maintaining collaboration with the healthcare team.
• Exceptional communication and patient education skills.
• Ability to travel to patient homes within Manhattan, Brooklyn, Bronx, and Queens .
