• DCH - Travel: Care Manager RN
• The Care Manager works collaboratively with interdisciplinary and healthcare team members to facilitate patient care through effective utilization and monitoring of healthcare resources.
• Assumes a leadership role to achieve safe discharge planning along with desired clinical and financial outcomes.
• Coordinates care and services through the acute care episode and across the continuum.

Job Requirements

• Associated degree in nursing
• 2 years experience required
• BSN and or related bachelor’s degree in a healthcare field preferred
• Any relevant experience in utilization and/or case management.
• Registered Nurse State of Alabama or compact license (No Temp Licenses accepted)

Schedule Information

• Must be willing to float to other units and hospitals
• Guaranteed Hours: 36
