HealthCare Support is actively seeking a Nurse Practitioner (NP) to fill an opening with one of America's largest physician enablement companies. This is a full-time opportunity, field based, and offering an extremely comprehensive benefit package including unlimited PTO.

Daily Responsibilities Nurse Practitioner:

• Deliver preventive care to members in the comfort of their home
• Complete a comprehensive review and assessment of a member's medical history, physical health, environment and psycho-social status.
• Document progress notes, prepare member workups and encounter summaries.
• Help at-risk members transition from the hospital and other facilities to their own home.
• Create personalized care plans in coordination with the member's primary care physician.
• Shift: Monday - Friday, 8:00 a.m. - 5:00 p.m.

Required Qualifications for Nurse Practitioner:

• Must be based in the state of New Jersey
• Certified as an Advanced Nurse Practitioner in specialty areas
• Hold current unencumbered NJ Licensure as a Registered Nurse
• Active DEA and CDS registration
• Minimum of two (2) years’ experience as an Advanced Registered Nurse Practitioner
• Master’s degree in advanced clinical practice nursing or graduate of post Master’s certificate program for Nurse Practitioner
• Current provider card in Basic Life Support (BLS)
• Willingness to travel to the member’s home and therefore must have reliable transportation

Preferred Qualifications for the Nurse Practitioner:

• Experience working with geriatric patients
• Home care experience

Pay Details: $117k - $155k a year + 10% annual bonus
