Portages Health is a Minnesota-based, physician-owned digital behavioral healthcare company. We have created our own collaborative model of care in tandem with a novel and highly effective technology platform to assist diligent providers in delivering the very best in compassionate, evidence-based care. We are seeking high caliber mental health therapists that we can support to deliver the best care you have ever provided.
The Role:
You will work in partnership within a multidisciplinary team of psychiatry providers, therapists, coaches, and care navigators and each patient's primary care providers. You will guide individuals through the complexities of emotional and psychological well-being. You will offer personalized support and tailored strategies to help clients develop coping skills, enhance self-awareness, and ultimately, achieve a healthier mental state. You and the team collaboratively guide patients with mental illness and/or substance use disorders, who may also have co-morbid physical health conditions, on their personalized journeys to improved mental health and wellbeing. Work is onsite 5days/week.
