Medicaid Care Coordinator Compensation
• The pay for this position is up to $20 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available to full-time employees with 1 year of service on our eligibility dates
Medicaid Care Coordinator Highlights
• Schedule: Monday–Friday, 8am–5pm CST (occasional flexibility past 5pm)
• Remote position — must reside in or near San Antonio, TX
• In-office meetings and team-building activities occur every 2–3 months
• Optional overtime is available but not mandatory
• Contract-to-hire opportunity
Medicaid Care Coordinator Responsibilities
• Conduct telephonic assessments with members or parents to identify unmet needs, barriers to health, and available services
• Develop and coordinate individualized care and service plans for long-term care members
• Collaborate with members, caregivers, and providers to ensure timely access to needed resources
• Monitor and adjust care plans based on changing member needs and outcomes
• Provide education on health services, community resources, and available benefits
• Maintain documentation in compliance with all federal, state, and company standards
• Participate in occasional in-office meetings and training sessions
Medicaid Care Coordinator Requirements
• High School Diploma or GED required
• Bachelor’s degree in Human Services or related field preferred
• 1+ year of experience in healthcare coordination, case management, or related field
• Strong computer proficiency (Microsoft Office and multiple systems navigation)
• Excellent communication and decision-making skills
• Must have reliable high-speed internet and quiet, professional home workspace
• Attendance is mandatory for the first 90 days
Medicaid Care Coordinator Preferences
• Experience working with Medicaid and members with special healthcare needs
• Bilingual (English/Spanish) a plus
• Self-motivated, quick learner, organized, and proactive in communication
